Nature of telehealth

Telehealth uses electronic communications to allow a patient and a licensed provider in different locations to exchange health information and provide evaluation, consultation, education, care management, and treatment. Encounters may be live, asynchronous, or a combination.

Material risks and limitations

Alternatives and choice

You may decline telehealth and seek available in-person care. You may withdraw telehealth consent prospectively by notifying the provider, subject to completing clinically necessary transition or emergency steps. Declining marketing messages does not affect eligibility for care.

Independent providers and pharmacies

The treating provider is independently responsible for professional judgment, informed consent, diagnosis, treatment, and prescribing. A licensed pharmacy independently evaluates and fulfills valid prescriptions. Nuveena provides technology and administrative coordination and does not practice medicine or pharmacy.

Privacy and recording

Telehealth information is handled according to applicable privacy law and the provider's and pharmacy's notices. Do not record a session unless every participant gives permission and recording is lawful.

Emergencies

Telehealth through Nuveena is not emergency care. Call 911 or go to the nearest emergency department for severe or rapidly worsening symptoms.

Patient acknowledgment

By electronically accepting during clinical intake, the patient confirms that the patient read this consent, had the opportunity to ask questions, understands the benefits, risks, alternatives, privacy considerations, and limited role of Nuveena, and voluntarily consents to telehealth. Acceptance should be stored with the version, date, time, and identity of the consenting patient.
